What the request form collects

The form asks for your name and phone number, and optionally your email address, what kind of crash it was, your county or ZIP code, whether you already have a lawyer, and a few words about the crash. Please do not send payment details, ID numbers or medical records through the form.

Consent records

Every form carries a consent checkbox that is never ticked in advance. When you send a request, the site stores a record of it: the date and time, your IP address and browser details, the exact consent text shown, the page you sent it from, the form values and a unique request number. This record is kept as proof of what you agreed to. Consent to be contacted is not a condition of getting a referral.

Calls and call recording

The phone number on this site is a call tracking number. Calls through it may be recorded for quality and training, and a recorded message at the start of the call can say so before you are connected. By continuing the call, you agree to the recording. The phone service logs the number a call came from, the time and the length of the call. Calls are connected to a participating independent attorney’s office, which hears what you tell it.

Sharing with participating attorneys

Your request is emailed to the operator, who passes it to a participating independent attorney’s office so that office can contact you about your request by phone call or text message, as the consent text describes. The operator may use a call and lead routing service to do this. Your details are shared only as this policy describes. If you hire an attorney, that attorney’s own records and policies apply to what you share with them.

Tracking technology

Like most websites, the site’s server keeps standard logs, such as IP addresses, browser types and the pages requested, to run and protect the site. The site may use cookies needed for the site to work. Form details and call details are not sent to advertising pixels or analytics tools.

Text messages

If you consented, a participating attorney’s office or the operator may contact you by text message about your request. Your carrier’s message and data rates may apply. Reply STOP to opt out.

Your choices

You can ask to see, correct or delete the details you sent, or withdraw your consent to be contacted. Use the contact page or email info@caraccidentlawyerfortwayne.com, and say which request is yours (the name and phone number you used).

Changes

If the way the site handles details changes, this page will be updated, and the new version applies to requests sent after the change.
